The Remember-I’m-Me pocket chart is a fantastic solution for communicating on behalf of loved ones with dementia. The illustrated symbols each represent a different need so that the chart can be quickly scanned for information by anyone providing care at the time. There are symbols for all the important stuff like family contacts and medicines, as well as creature comforts such as how they take their tea.

The Remember-I’m-Me charts have been designed to do exactly that; family can communicate personal information such as preferred names, hobbies and favourite foods so the carer can quickly build a personal relationship. Created by Zoe Harries, carer to her late husband who lived with dementia at home for 5 years, these charts were made with the individual, not the patient, in mind.
